Job Summary

A company is looking for an LPN Care Navigator to manage patient care and provide health coaching remotely.

Key Responsibilities

• Manage a panel of 400-450 patients and conduct monthly outreach for continuity of care
• Collaborate with clinical teams and caregivers to address patient needs and support care coordination goals
• Advocate for patients by listening to concerns and communicating solutions to their care circle

Required Qualifications

• Active Licensed Practical Nurse or Licensed Vocational Nurse license with a multi-state compact license
• Must reside in the U.S
• Minimum 2 years of clinical experience
• Experience working with EMR systems and familiarity with chronic condition management
• Intermediate to advanced computer skills and ability to navigate multiple systems
