Key Responsibilities

• Provide triage and assessment for hospice patients via remote communication
• Coordinate care and communicate effectively with patients and their families
• Document patient interactions and care plans in the electronic health record

Required Qualifications and Education

• Graduated from an approved school of professional nursing
• Compact RN License required
• Experience in hospice or palliative care preferred
• Availability to work part-time on weekends and one weeknight
• Basic computer skills necessary for documentation and communication
