Job Summary

A company is looking for a Clinical Nurse, Case and Disease Management.

Key Responsibilities

• Conduct member assessments and develop individualized care plans
• Monitor and evaluate the effectiveness of care plans and intervene as necessary
• Act as a liaison among members, providers, and support systems to ensure coordinated care

Required Qualifications

• Associate degree in nursing
• 3 years of full-time direct patient care clinical experience in transplant, oncology, or medical/surgical
• Valid and active Registered Nurse (RN) license in Missouri and Kansas
• Case Management Certification within 3 years of hire or transfer to the role
• Thorough knowledge of URAC and NCQA guidelines and state and federal regulations
