Missouri Licensed Care Coach

Job Summary

To support patient care continuity, the full-time Missouri Licensed Care Coach will facilitate patient outreach, coordinate care management programs, and assist patients in navigating their health needs, with a schedule of Monday to Friday from 8 a.m. to 4:30 p.m., including remote work with occasional onsite training.

Key responsibilities

• Provides support to ambulatory care management programs, conducting patient outreach and reviewing discharge instructions
• Identifies and manages patients' care needs according to established protocols and department policies
• Assesses barriers to care and coordinates assistance to address patient concerns

Required qualifications

• High School Diploma or GED
• 2-5 years of relevant experience
• Licensed Practical Nurse or Registered Medical Assistant certification
