Missouri Licensed Care Manager
Job Summary
A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.
Key Responsibilities
- Evaluate member needs and recommend care plans to achieve optimal health outcomes
- Coordinate with providers and community resources to address members' unmet needs
- Monitor member progress and revise care plans as necessary to meet individual goals
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in nursing or care management
- RN - Registered Nurse State Licensure and/or Compact State Licensure required
- Must reside in the state of Missouri
- Availability to work Monday - Friday, 8 AM - 5 PM Central Time Zone
