Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members.

Key Responsibilities

• Evaluate member needs and recommend care plans to improve healthcare outcomes
• Coordinate care between members, families, and providers to ensure appropriate services
• Monitor member progress and update care plans as necessary to meet goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• Registered Nurse (RN) state licensure or compact state licensure required
