Missouri Licensed Care Manager
Job Summary
A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.
Key Responsibilities
- Evaluate member needs and recommend care plans to address barriers and social determinants of health
- Collaborate with providers to develop ongoing care plans and ensure access to necessary resources and services
- Monitor member progress and provide education to members and families regarding healthcare options and benefits
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
- Must reside in the state of Missouri
