Missouri Licensed Care Manager
Job Summary
A company is looking for a Care Manager (Behavioral Health) to develop and facilitate care management activities for members with mental and behavioral health needs.
Key Responsibilities
- Evaluate member needs and facilitate care/service plans for optimal outcomes
- Develop ongoing care plans for members with high acuity and coordinate with providers and community resources
- Monitor care plans and provide education to members and families regarding treatment options and healthcare benefits
Required Qualifications
- Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
- 2 - 4 years of related experience in behavioral health
- Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN with BH experience
- Must reside in the state of Missouri
