Missouri Licensed Care Manager
Job Summary
To support a diverse population, the full-time Missouri Licensed Care Manager will develop and facilitate complex care management activities for members with physical health needs, ensuring high-quality healthcare outcomes while working remotely.
Key responsibilities
- Evaluate member needs and barriers to care, recommending personalized care plans and interventions
- Develop ongoing care plans and collaborate with providers to address unmet needs and ensure timely access to services
- Monitor member progress towards care plan goals and provide education on healthcare options and benefits
Required qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Current RN - Registered Nurse state licensure or compact state licensure
- Experience with the foster care population is preferred
- Knowledge of physical health and behavioral health is a plus
