Job Summary
A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members.
Key Responsibilities
- Evaluate members' needs and barriers to care, recommending plans for optimal outcomes
- Develop ongoing care plans and collaborate with providers to address members' unmet needs
- Provide education to members and their families on healthcare options and resources
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in nursing or care management
- Registered Nurse (RN) state licensure or Compact State Licensure required
