Missouri Licensed Care Manager RN

Job Summary

To support the health of members, the full-time Missouri Licensed Care Manager RN will develop and facilitate complex care management activities for NICU patients remotely, focusing on personalized care plans and education for members and their families.

Key responsibilities

• Evaluates member needs and barriers to care, recommending plans for optimal outcomes
• Develops ongoing care plans in collaboration with providers and identifies community resources
• Coordinates care between members, families, and providers to ensure person-centered services

Required qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2-4 years of Level III/IV bedside NICU experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
