Job Summary

A company is looking for a Telephonic Care Navigator.

Key Responsibilities

• Completes telephonic outreach to DSNP/CSNP members and assists with care gaps
• Educates members on healthcare access and links them to community resources
• Maintains schedule adherence while providing excellent customer service

Required Qualifications

• High School Diploma / GED or equivalent work experience
• Must be 18 years of age or older
• 1+ years of call center or telephonic customer service experience
• 1+ years of professional experience using telephone and computer in an office setting
• 1+ years of experience in healthcare, insurance, social work, or community outreach
