Job Summary
To support pediatric behavioral health needs, the full-time remote In-Home Services Case Manager will manage a caseload of clients aged 4 to 18, focusing on treatment planning, community resource linkage, and family support.
Key responsibilities:
- Manage case management services for children and families in community settings
- Implement treatment plans developed by Master level clinicians and develop crisis plans
- Coordinate care with families, schools, and community resources while providing 24/7 on-call availability
Required qualifications:
- Bachelor's degree in Social Work, Psychology, Counseling, or a related field
- At least 1 year of applicable experience in case management
- Clean driving record with applicable vehicle insurance
