Job Summary

To deliver exceptional patient-centered care in a virtual setting, the full-time Missouri Licensed Clinic RN will assess patient needs, provide triage support, coordinate referrals, and manage follow-up care while working remotely after training.

Key responsibilities

• Assess and manage patient conditions, including discharge planning and medication education
• Coordinate scheduling and transitional care management, ensuring seamless patient experiences
• Maintain nursing standards and participate in quality improvement initiatives within a multi-disciplinary team

Required qualifications

• Missouri Board of Nursing RN or Nurse Licensure Compact multi-state RN
• Experience in patient assessment and triage in a clinical setting
• Knowledge of medications, including actions and side effects
• Ability to work collaboratively within a healthcare team
• Commitment to continuous improvement and evidence-based practice
