Job Summary

Key Responsibilities

• Assist patients through home visits and by attending medical appointments
• Review referral forms and determine project participation appropriateness
• Provide support and education to members to help them adhere to medical recommendations

Required Qualifications

• High School Diploma
• 1-3 years' experience working with underserved populations
• Experience in navigating health and social service systems
• Demonstrated ability to support and motivate patients
• Valid driver's license and completion of community health worker certification within one year of hire
