Missouri Licensed Discharge Care Nurse

Job Summary

Working remotely, the full-time Missouri Licensed Discharge Care Nurse will implement telephonic case management interventions for high-risk members, assess discharge needs, and coordinate care with clinical teams to enhance health outcomes.

Key responsibilities

• Evaluate member discharge needs and develop readmission prevention plans
• Collaborate with providers and members to ensure coordinated post-discharge care
• Document discharge activities and participate in team meetings and education discussions

Required qualifications

• Bachelor of Science in Nursing
• Active, unrestricted Missouri RN License
• 5+ years of recent experience in the inpatient acute setting
• 2+ years of experience in discharge planning
• Experience with multiple health insurance products, including regulatory compliance
