Missouri Licensed Discharge Care Nurse

Job Summary

To support high-risk members, the remote Missouri Licensed Discharge Care Nurse will implement telephonic case management interventions, assess discharge needs, and coordinate care post-discharge while collaborating with clinical teams.

Key responsibilities

• Evaluate member discharge needs and develop readmission prevention plans
• Collaborate with providers and members to ensure coordinated care after discharge
• Document discharge activities and participate in team meetings and education discussions

Required qualifications

• Bachelor of Science in Nursing
• Active, unrestricted RN License in the state of residence
• 5+ years of recent experience in the inpatient acute setting
• 2+ years of experience in discharge planning
• Experience with multiple health insurance products, including regulatory requirements
