Job Summary

A company is looking for a Health Home Care Coordinator Supervisor.

Key Responsibilities

• Provide leadership, guidance, and support to Care Coordinators for high-quality service delivery
• Support the design and improvement of data collection processes and reporting systems
• Analyze reports and data to identify trends and opportunities for improvement

Required Qualifications, Training, and Education

• High School/GED required; Bachelor's degree in healthcare management or related field preferred
• At least two years' experience in health home or data reporting
• Proficient knowledge of Microsoft Office suite applications preferred
• Significant experience in care coordination or related healthcare roles
• Leadership or supervisory experience preferred
