Job Summary

A company is looking for a Mental Health Case Manager - Therapist for Virtual Collaborative Care.

Key Responsibilities

• Provides case management and therapy to enhance patients' psychosocial functioning
• Coordinates interdisciplinary care and oversees comprehensive discharge planning
• Conducts behavioral health assessments, therapy, crisis intervention, and treatment planning

Required Qualifications, Training, and Education

• Master's degree in Social Work, Counseling, or related human-service field
• Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) in Missouri
• Minimum 2-3 years of behavioral health experience preferred
• Proficient computer skills required
