Job Summary

A company is looking for a Mental Health Case Manager II-Therapist (Remote).

Key Responsibilities

• Provides case management and therapy for patients and families to enhance psychosocial functioning
• Coordinates interdisciplinary care and oversees safe, comprehensive discharge planning
• Conducts behavioral health assessments, therapy, crisis intervention, and ongoing treatment planning with accurate documentation

Required Qualifications

• Master's degree in Social Work, Counseling, or a related human-service field
• Licensure as a Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) in Missouri
• Minimum 2-3 years of Behavioral Health experience preferred
• Proficient computer skills
