Job Summary

A company is looking for a Pediatric Advanced Practice Provider.

Key Responsibilities

• Manage a panel of complex pediatric patients, overseeing care plans and ensuring comprehensive care coordination
• Work within a multidisciplinary team to provide wrap-around care in a virtual-first model
• Empower families by educating and supporting them in navigating their child's health journey

Required Qualifications

• Minimum 5 years' experience with a pediatric population, preferably in Community Based Health or Complex Care
• Completion of an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) with an unrestricted state license and board certification
• Experience with children with complex medical needs
• Experience with Medicaid and/or Community health preferred
• Bilingual Spanish is strongly preferred
