Job Summary

A company is looking for an Integration Nurse Care Coordinator.

Key Responsibilities

Coordinate hospital follow-up activities, including medication reconciliation and discharge documentation review
Ensure accurate and timely updates within the electronic health record and communicate key information to care team members
Monitor reports and data to identify gaps in care, missed follow-ups, and opportunities for improvement

Required Qualifications, Training, and Education

High School/GED required
Practical Nursing certificate required
Must possess and maintain a current, active Missouri Licensed Practical Nurse (LPN) license
Experience and efficiency with healthcare technology software preferred
