Missouri Licensed Social Worker

Job Summary

To support a growing Advanced Primary Care team, the full-time Missouri Licensed Social Worker will assess patients' needs, develop individualized care plans, and connect patients with community resources in a hybrid work environment.

Key Responsibilities

• Assess patients' psychosocial, environmental, financial, and support needs
• Develop and implement individualized care plans that address barriers to health and wellness
• Collaborate with interdisciplinary team members to coordinate care and improve patient outcomes

Required Qualifications

• Bachelor's degree in Social Work required; Master's degree preferred
• Current social work licensure, if required by state regulations
• Previous experience in healthcare, case management, or community health preferred
• Strong knowledge of community resources and social service programs
• Ability to manage multiple priorities in a fast-paced, collaborative environment
