Job Summary

To deliver high-quality, preventive-focused care through telehealth, the full-time Missouri Licensed Telehealth Physician will provide routine visits to Medicare patients, manage personalized care plans, and collaborate with an integrated care team, all while working remotely without on-call duties or weekend shifts.

Key responsibilities

• Provide routine telehealth visits with Medicare patients, focusing on E/M and Chronic Care Management
• Create and manage personalized care plans for ongoing conditions while ensuring thorough documentation in the EHR
• Collaborate with the integrated care team, including nurse case managers and NPs, to engage patients in preventive care

Required qualifications

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license; multi-state licensure is a plus
• Credentialed with major insurance plans such as UHC, Aetna, and Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
