Job Summary

Delivering high-quality, preventive-focused care, the full-time Missouri Licensed Telehealth Physician will provide routine telehealth visits, manage personalized care plans, and collaborate with an integrated care team, all while working remotely Monday through Friday from 9 am to 6 pm.

Key responsibilities

• Provide routine telehealth visits with Medicare patients, focusing on E/M and Chronic Care Management
• Create and manage personalized care plans for ongoing conditions while ensuring thorough documentation in the EHR
• Engage patients in preventive care and collaborate with the integrated care team to improve health outcomes

Required qualifications

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Missouri medical license, with residence not required
• Minimum of 1 year of physician experience in family medicine or internal medicine
• Experience in preventive care, senior health, or chronic condition management is preferred
• Comfortable practicing via telehealth and using EHR systems
