Job Summary
To support a growing Virtual Anticoagulation Specialty Clinic, the full-time Montana and Colorado Licensed Clinical Pharmacist will manage anticoagulation therapy, collaborate with healthcare teams for effective medication monitoring, and provide drug information to patients and providers, all while working primarily remotely with a Monday to Friday schedule.
Key responsibilities
- Manage anticoagulation therapy under collaborative practice agreements for patients in Colorado and Montana
- Collaborate with the healthcare team to ensure safe, high-quality care and effective medication monitoring
- Provide counseling and medication education to patients, ensuring adherence and understanding of their treatment
Required qualifications
- Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution
- License to practice pharmacy in Montana and Colorado or ability to obtain licensure in both states
- Current knowledge of state and federal pharmacy regulations
- Immunization certification if applicable to practice site needs
- Previous experience in an integrated healthcare delivery system is preferred
