Job Summary

A company is looking for a Clinical Navigation, Family Nurse Practitioner (Per Diem).

Key Responsibilities:

• Assess patient needs, gather clinical history, and provide clinical guidance while navigating them to appropriate resources
• Deliver high-quality clinical services and monitor care quality to meet patient satisfaction and clinical effectiveness benchmarks
• Collaborate on projects and workflow changes to enhance efficiency and improve member experience

Required Qualifications:

• Licensed NP/PA provider in good standing with state board
• Maintains board certification
• At least 3 years of NP/PA experience in a clinical setting
• Comfortable providing care to all age groups
• Must be in PST or MST time zone
