Job Summary

A company is looking for a Cardiometabolic Nurse Practitioner to manage chronic disease care for vulnerable seniors.

Key Responsibilities

• Conduct thorough assessments and virtual visits for patients with cardiometabolic conditions
• Formulate diagnoses and develop individualized treatment plans, including medication management
• Manage prescription refill requests and collaborate with the care team to ensure continuity of care

Required Qualifications, Training, and Education

• APRN with 3+ years of outpatient clinical practice experience, focusing on hypertension and hyperlipidemia
• Minimum of 2 years in primary care or a subspecialty with emphasis on cardiovascular disease prevention
• Proficiency in telehealth technology and electronic health records (EHR)
• Master's or Doctoral degree in Nursing with an active RN and APRN license required
• Multi-state licensure and active BLS certification required at hire
