Job Summary

To deliver exceptional diabetes care, the full-time salaried Multi-State Diabetes Care Provider will conduct virtual consultations, develop individualized treatment plans, and collaborate with an interprofessional team to enhance patient outcomes in a remote setting.

Key responsibilities

• Conduct thorough assessments of diabetes patients through virtual consultations, including diagnostics and treatment planning
• Formulate accurate diagnoses and individualized treatment plans, including medication management and lifestyle modifications
• Collaborate with care team members to coordinate holistic care and ensure continuity of care for patients

Required qualifications

• Master's or Doctoral degree with an active RN and APRN or PA license
• Minimum of 2 years of experience in diabetes management and education
• Proficiency in telehealth technology and electronic health records (EHR)
• Multi-state licensure with a willingness to obtain additional licensure as needed
• Experience working in outpatient clinical practice, ideally in an endocrinology or diabetes-focused clinic
