Job Summary

A company is looking for a Diabetes Specialty Care Nurse Practitioner.

Key Responsibilities

• Conduct assessments and develop treatment plans for patients with diabetes through virtual consultations
• Collaborate with the care team to ensure continuity and holistic care for patients
• Participate in quality improvement initiatives and maintain accurate patient records

Required Qualifications, Training, and Education

• APRN with 5+ years in outpatient clinical practice, ideally in endocrinology or diabetes
• Minimum of 2 years in diabetes management and education
• Proficiency in telehealth technology and electronic health records (EHR)
• Multi-state licensure and willingness to obtain additional licensure as needed
• Master's or Doctoral degree in Nursing with a specialization in diabetes care
