Job Summary
To deliver exceptional preventive cardiovascular care to members with hypertension, the full-time Multi-State Licensed Advanced Practice Provider will conduct virtual consultations, develop individualized treatment plans, and collaborate with an interdisciplinary team in a remote setting.
Key responsibilities:
- Conduct thorough assessments and develop treatment plans for patients with hypertension through virtual consultations
- Collaborate with an interdisciplinary care team to coordinate holistic, patient-centered care
- Utilize and provide feedback on the electronic health information system while maintaining accurate patient medical records
Required qualifications:
- Multi-state licensure as an Advanced Practice Provider with at least 5 years of outpatient clinical practice experience
- A minimum of 2 years concentrated in primary care or a subspecialty with a focus on hypertension and lipid management
- Proficiency in telehealth technology and electronic health records (EHR)
- Active BLS certification at the time of hire
- Proficiency in English and Spanish is preferred
