Job Summary
To deliver exceptional preventive cardiovascular care, the full-time Multi-State Licensed Advanced Practice Provider will conduct virtual consultations, manage hypertension, and collaborate with an interdisciplinary team while working remotely during standard business hours.
Key responsibilities
- Conduct thorough assessments and develop individualized treatment plans for patients with hypertension through virtual consultations
- Collaborate with an interdisciplinary care team to coordinate holistic, patient-centered care and ensure continuity of treatment
- Utilize and provide feedback on the electronic health information system while maintaining accurate patient medical records
Required qualifications
- An active RN and APRN or Physician Assistant license, with a willingness to obtain and maintain multiple state licenses
- 5+ years of outpatient clinical practice, including 2+ years in primary care or a relevant subspecialty focused on hypertension management
- Experience with medication management and lifestyle modifications for cardiovascular disease prevention
- Ability to work Monday-Friday from 8:30am-5:00pm MST or PST, with consideration for EST and CST
- Familiarity with telehealth technology and electronic health records (EHR) is preferred
