Multi-State Licensed Advanced Practice Provider

Job Summary

To support a growing virtual care initiative, the full-time Multi-State Licensed Advanced Practice Provider will deliver remote text and video-based acute care, including preventive care, sick visits, and chronic condition management, while maintaining strong patient relationships across FL, NJ, NY, MA, and NLC.

Key responsibilities:

• Provide comprehensive video-based clinical care, including pediatrics and mental health
• Support physicians with Epic inbox management and educate patients on treatment plans
• Collaborate with Care Team Members to drive high-quality patient outcomes

Required qualifications:

• 5+ years of clinical practice experience as a Family Nurse Practitioner or Physician Assistant in a Primary Care setting
• Active licensure in FL, NJ, NY, MA, and/or NLC
• Minimum 1 year of experience using Epic EMR
• Experience in primary care and behavioral health, treating both acute and chronic conditions
• Tech-savvy with telemedicine experience and a clean background and medical malpractice history
