Job Summary

A company is looking for a Clinical Pharmacist, Pharmacy Outreach (Remote).

Key Responsibilities

• Counsel patients on medication adherence and address medication-related problems
• Collaborate with healthcare teams to optimize medication management workflows
• Participate in the design and evaluation of pilot programs to improve clinical operations

Required Qualifications

• Doctor of Pharmacy (PharmD)
• Active pharmacist license in at least two specified states
• Minimum of 3 years of experience in value-based care or ambulatory care
• In-depth knowledge of pharmacotherapy and pharmacy practice
• Willingness to obtain additional state licenses within 6 months of hire
