Multi-State Licensed Clinical Pharmacist
Job Summary
To support a growing clinical pharmacy practice, the full-time Multi-State Licensed Clinical Pharmacist will conduct telephonic medication consultations, manage medication reviews, and collaborate with healthcare providers in a remote setting.
Key Responsibilities
- Conduct telephonic medication consultations that meet/exceed production and quality metrics
- Complete annual medication reviews and pharmacotherapy assessments for patients
- Deliver evidence-based therapeutic recommendations and document clinical consultations
Required Qualifications
- Active, unrestricted pharmacist license in the state of primary residence, with willingness to obtain additional state licenses
- 5+ years of pharmacy practice experience, including 2+ years in Clinical Pharmacy Practice
- Experience with Medicare STARs & HEDIS Measures, including Medication Therapy Management
- Bilingual in Spanish and English
- High school diploma or equivalent; Doctor of Pharmacy degree preferred
