Multi-State Licensed Dietitian

Job Summary

To support a growing health management initiative, the full-time Multi-State Licensed Dietitian will provide telephonic nutrition counseling and case management services for individuals with chronic conditions, focusing on personalized care plans and collaboration with interdisciplinary teams.

Key responsibilities

• Conduct comprehensive nutrition assessments and develop individualized care plans aligned with medical conditions
• Deliver telephonic nutrition counseling to promote behavior change and improve self-management for chronic disease management
• Collaborate with care teams and document interventions while maintaining HIPAA compliance and privacy of member records

Required qualifications

• Registered Dietitian Nutritionist (RDN) with a Bachelor's degree in nutrition, Dietetics, or a related field; Master's degree preferred
• Minimum of 3 years of clinical nutrition experience, preferably in case management or wellness program coordination
• Ability to obtain multi-state licenses as required by business needs
• Knowledge of medical claims and coding (ICD-10, CPT, HCPCS)
• Proficiency in software applications including Microsoft Word, Excel, and Outlook
