Job Summary

A company is looking for a Functional Medicine Nurse Practitioner.

Key Responsibilities

Review patient health history intake forms and lab work to identify care plans
Conduct initial telehealth consultations to determine personalized HRT and treatment plans
Update patient charts and communicate effectively with other providers

Required Qualifications

Masters Degree, Post Master's Certificate, or Doctorate in Nursing
Current national certification in family practice through ANCC, AANP, or another recognized certification
Active state licenses in a minimum of 2 specified states
Unrestricted DEA license
3+ years of experience as a Nurse Practitioner in family practice or primary care (preferred)
