Job Summary
Providing telephonic care management for members with complex and chronic needs, the full-time Multi-State Licensed Nurse Case Manager will assess, develop, implement, coordinate, and evaluate care plans while working remotely with occasional in-person training sessions.
Key responsibilities
- Conduct assessments to identify individual needs and develop specific care management plans
- Implement care plans by facilitating authorizations and coordinating resources
- Monitor and evaluate the effectiveness of care management plans, making modifications as necessary
Required qualifications
- BA/BS in a health-related field and a minimum of 3 years of clinical experience
- Current unrestricted RN license in applicable state(s)
- Multi-state licensure required for servicing members in different states
