Multi-State Licensed Nurse Practitioner

Job Summary

A company is looking for a Part-Time Nurse Practitioner to manage patient care remotely.

Key Responsibilities

• Oversee monitoring of patient vitals, symptoms, and labs in collaboration with healthcare team
• Manage treatment plans and medications, focusing on guideline-directed medical therapy for chronic conditions
• Conduct regular virtual check-ins with patients to optimize adherence to treatment plans

Required Qualifications

• 5+ years of experience treating chronic diseases (T2D, Hypertension, CHF) in a medical setting
• Active multi-state compact RN license and Nurse Practitioner license in multiple states
• ANCC or AANP certification required
• Master's Degree as a Nurse Practitioner with willingness to expand state licensure
• Experience managing CHF with guideline-directed medical therapy
