Multi-State Licensed Nurse Practitioner

Job Summary

To support a growing telehealth initiative, the remote contract Multi-State Licensed Nurse Practitioner will conduct structured intake consultations, perform telehealth visits to establish medical necessity, and collaborate with care advocates to develop individualized care plans.

Key responsibilities

• Conduct initiating telehealth visits to assess medical necessity and identify high-risk conditions
• Document findings accurately in the platform, ensuring proper coding and use of SDOH Z-codes
• Collaborate with care advocates to finalize individualized care plans aligned with patients' needs

Required qualifications

• Completion of an accredited Nurse Practitioner program
• Active Nurse Practitioner or APRN license in good standing, licensed in multiple states
• 2+ years of experience in primary care, telehealth, or care management
• Familiarity with Medicare programs and SDOH documentation
• Proficiency with EMRs and telehealth workflows
