Multi-State Licensed Nurse Practitioner

Job Summary

To provide accessible and convenient care for members, the full-time hourly Multi-State Licensed Nurse Practitioner will manage medication management and outpatient treatment via Telehealth, focusing on obesity, hypertension, diabetes, and related conditions.

Key responsibilities

• Provide direct patient care by prescribing and supporting treatment plans in accordance with regulations
• Order laboratory tests, interpret results, and educate patients on medications and treatment expectations
• Maintain medical records and high-quality clinical documentation while engaging in follow-up communication with patients

Required qualifications

• Active, unrestricted NP license in 10+ states, ideally including PA, CA, NY, TX, FL, MD, VA
• Board Certified with experience in treating CHF, COPD, Diabetes, and Obesity preferred
• Demonstrated experience in an integrated clinical setting working with patients and providers
• Proficiency with EMR systems and experience with digital technology
• Ability to work independently with accountability for specific goals and objectives
