Job Summary
To deliver comprehensive patient care remotely, the full-time Multi-State Licensed Nurse Practitioner will provide holistic, evidence-based assessments, diagnoses, treatments, and health counseling for patients aged 18 months and above while collaborating with a dedicated healthcare team.
Key responsibilities
- Provide holistic, evidence-based care including assessment, diagnosis, treatment, and health counseling for patients
- Evaluate and manage primary care, acute, chronically ill, and transitional care patients, including health education and counseling
- Document patient care within an EHR and coordinate care with internal and external colleagues to ensure high standards of care
Required qualifications
- Current unrestricted licensure in FL, OH, MA, RI, DE, MD, or Washington D.C
- Completion of a Master's Degree level Family Nurse Practitioner program with National Board Certification
- Minimum of two years of medically relevant experience or equivalent
- Ability to work autonomously and collaboratively within a healthcare team
- Proficiency in information management and technology; bilingual skills may be required depending on the market
