Multi-State Licensed Nurse Practitioner

Job Summary

Providing remote patient care, the part-time Multi-State Licensed Nurse Practitioner will deliver holistic, evidence-based services, including assessment, diagnosis, and treatment for patients aged 18 months and above, while collaborating with a dedicated healthcare team during evening shifts.

Key responsibilities:

• Deliver holistic patient care through accurate assessment, diagnosis, and treatment, including health counseling and disposition planning
• Evaluate and manage primary care, acute, and chronic patients, providing healthcare education and counseling
• Document patient encounters in an EHR system and coordinate care with internal and external colleagues

Required qualifications:

• Current licensure in D.C., DE, FL, KS, KY, MA, MD, ME, NH, OH, RI, VT
• Minimum of two years of medically relevant experience or equivalent
• Completion of a Master's Degree level Family Nurse Practitioner program with National Board Certification
• Ability to work autonomously and effectively communicate in both verbal and written forms
• Proficiency in information management and technology
