Multi-State Licensed Nutritional Coordinator

Job Summary

To support patients with chronic conditions, the full-time Multi-State Licensed Nutritional Coordinator will provide remote nutritional education and care coordination, managing a panel of 300-350 patients through outreach and tailored health coaching.

Key responsibilities

• Conduct monthly nutritional outreach and support through 50-60 daily patient calls
• Educate patients on dietary modifications and reinforce adherence to care plans
• Collaborate with clinical teams to address nutritional needs and escalate patient concerns as necessary

Required qualifications

• Minimum 2+ years of experience in a healthcare role such as care coordination, health coaching, or as a Medical Assistant with a nationally recognized certification
• Active Licensed Practical Nurse or Licensed Vocational Nurse license with a multi-state compact license
• 2+ years of clinical nutrition counseling or dietary care coordination experience
• Experience with Electronic Medical Records (EMR) and computer navigation
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability
