Job Summary
To support a growing palliative care initiative, the Multi-State Licensed Palliative Care Nurse Practitioner will provide specialty outpatient care via video visits, oversee interdisciplinary case conferences, and educate team members on cancer-related symptom management and advance care planning, with the flexibility to work remotely or in a hybrid model.
Key responsibilities
- Conduct virtual outpatient appointments for members with serious illnesses as part of an interdisciplinary team
- Lead and direct interdisciplinary case conferences, ensuring individualized care plans are developed and monitored
- Provide education on palliative care topics to Community Health Workers and Oncology Nurses, focusing on symptom management and advance care planning
Required qualifications
- At least 3 years of nurse practitioner experience, with a minimum of 2 years in palliative care
- Advanced certification as a hospice and palliative nurse practitioner (ACHPN) and an unrestricted nurse practitioner license
- Experience in an outpatient oncology setting is preferred
- Proficiency with technology, including video conferencing and electronic health records
- Ability to thrive in a fast-paced, startup environment and manage multiple priorities effectively
