Job Summary

To support a growing palliative care team, the remote Multi-State Licensed Palliative Care Nurse Practitioner will conduct specialty outpatient appointments via video, oversee interdisciplinary case conferences for members with advanced serious illness, and provide education on palliative care management to Community Health Workers and Oncology Nurses.

Key responsibilities
  • Conduct specialty palliative care outpatient appointments via video visits
  • Oversee interdisciplinary case conferences and guide the creation of individualized care plans
  • Educate team members on outpatient management of cancer-related symptoms and advance care planning
Required qualifications
  • At least 3 years of nurse practitioner experience, with 2 years in palliative care, preferably in outpatient oncology
  • Advanced certification as a hospice and palliative nurse practitioner (ACHPN)
  • Compact RN license and unrestricted nurse practitioner license, with a willingness to obtain additional state licenses
  • Experience in a startup or tech-forward environment is preferred
  • Proficiency in using technology, including video conferencing and electronic health records
