Multi-State Licensed RN Liaison

Job Summary

Seeking a dynamic Multi-State Licensed RN Liaison, this full-time remote position will manage communication with patients and referral sources, coordinate care across departments, and perform eligibility reviews for continuum of care services, with a schedule of three weekdays and weekends focused on hospice support.

Key responsibilities:

• Facilitate communication with patients and families regarding continuum of care services
• Coordinate care between various departments and manage urgent hospice referrals on weekends
• Conduct eligibility and chart reviews to assess appropriateness for services

Required qualifications:

• Graduate of an accredited school of nursing
• Must obtain licenses for MO, IL, OK, and WI within 6 months of starting
• Basic Life Support HealthCare Provider (BLS HCP) certification from the American Heart Association
• Experience in Hospice, Case Management, or Home Health preferred
• No prior experience required
