Multi-State Nurse Practitioner

Job Summary

A company is looking for a Nurse Practitioner to support patients in their remote care journey.

Key Responsibilities

• Oversee monitoring of patient vitals, symptoms, and labs in collaboration with RNs and MDs
• Manage treatment plans and medications, focusing on guideline-directed medical therapy for chronic conditions
• Conduct regular virtual check-ins with patients to optimize treatment adherence and review lab results

Required Qualifications

• 5+ years of experience treating chronic diseases (T2D, Hypertension, CHF) in a medical setting
• Active multi-state compact RN license and Nurse Practitioner (NP) license in Texas and additional states
• ANCC or AANP certification required
• Master's Degree as a Nurse Practitioner with willingness to expand state licensure
• Experience managing CHF with guideline-directed medical therapy preferred
