Job Summary

A company is looking for a Nurse Practitioner for the National After-Hours Team - PRN - with licensure in IA, MO, KS, and OH.

Key Responsibilities

Provide telephonic support and medical care for residents and facility staff after-hours
Utilize a telephonic platform to coordinate care between facilities, hospitals, and clinics
Conduct comprehensive assessments and document encounters in the EMR, ensuring compliance with CMS requirements

Required Qualifications

Master's Degree as a Nurse Practitioner
Current unrestricted NP license in Iowa, Missouri, Kansas, or Ohio (must be licensed in 2 states)
Nurse Practitioner national certification as ANP, FNP, or GNP
3+ years of experience as a Nurse Practitioner
Background in acute and chronic disease management
