Multi-State Nurse Practitioner

Job Summary

Multi-State Nurse Practitioner, this is a 1099 contractor position requiring a minimum of 20 hours per week of remote consulting services, focusing on reviewing patient data, developing care plans, and ensuring treatment plans are signed off for Remote Patient Monitoring and Chronic Care Management services.

Key Responsibilities

• Review patient data reports for accuracy and appropriateness based on medical history and care plan goals
• Develop and recommend care plans for treating providers based on patient assessments
• Identify patients at risk for declining health and support clinical reviewers in enhancing their decision-making skills

Required Qualifications

• Advanced Practice Nurse practitioner education and certification
• Licensed as a Nurse Practitioner in at least 5 states
• Active and non-restricted nursing license
• Knowledge in chronic care management and remote patient monitoring preferred
• Proficient with Microsoft Office and video conferencing platforms
